I understand and acknowledge that the decision to work out and exercise is an individual choice and the risk of injury or death will depend upon my skill, training, fitness level, and health. I further understand and acknowledge that this program can be a test of my physical abilities and can carry with it the potential of injury and even death. By checking the box below, I hereby agree to assume all of the risks, known or unknown, of participating in the program, including performing the exercises depicted therein. The producers and sponsors of this program make no representation, warranty or guarantee whatsoever, including expressed or implied, with respect to the safety or effectiveness of the methods, techniques, exercises and other things described or demonstrated in this program. The producers and sponsors of this program disclaim all liability for loss, injury or death in conjunction with the methods, techniques, exercises, and advice given herein. In that regard, by checking the box below, I agree to assume all of the risks, whether known or unknown, of participating in the program and to further release and hold harmless all persons, including the producers and sponsors of this program, who might otherwise be liable to me for damages. I understand and agree that this waiver, release, and assumption of risk is to be binding on my heirs and assigns. I understand and agree that this waiver, release, and assumption of risk shall be construed in accordance with the laws of the State of Arizona and that specifically embraces each and every part of the program purchased. In checking the box below, I acknowledge and represent that: I have read the foregoing Waiver, Release of Liability and Hold Harmless Agreement, understand it, and agree to sign it voluntarily as my own free act and deed; no oral representation, statements, or inducements, apart from the foregoing written agreement, have been made; and I am at least eighteen (18) years of age and fully competent. Further, by checking the box below, I acknowledge and understand that, once the program is purchased, there will be no refunds under any circumstances.
